Intake Forms –I will ask you to begin by filling out a lengthy document for new patients. This may take 10-30 minutes. If you are able to mail it back to me 5 DAYS before the appointment, you will get much more from the first visit. If mailing it within that time frame is not possible, then be assured I will still study the material inside after the visit. If you are uncomfortable with answering some of the questions in the history form, leave them blank.
Logistics, Parking, Punctuality —Eating lightly before the appointment, with a snack packed for after our appointment, is wise. Kwan Yin Healing Arts is located west of NW 23rd street, up the hill on the South side of Flanders Street. You will find parking available in the lots east of the building and within the courtyard. Please do NOT park under the building, as those spaces are reserved for the employees. If your appointment will last longer than 2 hours, ask the front desk for a parking pass. Otherwise, you may park in the lot for 2 hours for free. If there are no spaces available when you arrive, it is best to search along NW 23rd Street or North of Kwan Yin Healing Arts (NW 24th, Glisan, Everett, Hoyt, etc). This rarely happens, but it is a possibility. If your appointment is before 9 am, there will not be a receptionist to welcome you. Please make yourself at home in the lobby, and feel free to empty your bladder in the restroom that is located down the hall. If you need to change your appointment at the last minute, and your appointment begins before 9 am, please call my mobile phone – 971-998-2196. Overall, I run on-time about 80% of the time, 10 minutes late about 15% of the time and more than 15 minutes late about 5% of the time.
Contact Information —You are welcome to call me on my mobile phone with questions. The best times to reach me are: Mondays and Fridays between 9am and 3pm, Tues-Thurs between 1:30-3 pm. If you are pregnant, or your child is my patient, you may call at any time of day, or night (if urgent). If the phone call runs longer than 15 minutes, the call then becomes a phone consult, billed at $60/hour for the total call time. Rescheduling —If your period comes early or late and interferes with receiving the Arvigo Techniques of Maya Abdominal Therapies™, please call or email me to problem solve scheduling. If you need to reschedule for other reasons, please honour our 24-hour cancellation policy. There are many people waiting to be seen. Giving me less than 24 hours warning prevents me from seeing another patient who needs treatment, and obstructs the flow of money coming in to run our clinic. I will have to charge you $55 for a missed appointment.
Appointments: My schedule consists of 45-minute time blocks, each one is $90 if paid on the Day of the Service. I can help you determine how much time will be both effective and cost effective. I advise 2hrs and 15 minutes for the first visit for there is much to do! If you have financial hardship, you may opt for a 45 minute or 1.5 hr visit, and then the first visit will be split over a few visits. For those with health insurance, please book a 2hr 15 min initial visit, and 1.5 hr follow-up visits. Pregnant women can receive the Arvigo Techniques of Maya Abdominal Therapies in 45 minutes, or 1.5 hr visit.
